I authorize the use of any photographic or electronic reproductions of my child for ay purpose as may be deemed appropriate by Mayer Community Church, including but not limited to print media and online media, such as the website or official social media accounts.
I acknowledge the church may choose not to use my media at this time, but may do so at its own discretion at a later date.
I also understand that once the image is posted on the church's website or social media, the image can be downloaded by any computer user, anywhere in the world. Therefore, I agree to indemnify and hold harmless the church, its trustees, pastor, deacons, its members and volunteers from any claims arising out of the use of my reproduction. The church reserves the right to discontinue use of any media without notice. I understand that my child may be identifiable from such photographic or electronic reproductions. I waive any rights, claims, liabilities or interest I may have to control the use of my child's identity or likeness in whatever media used.
